When we observe physical patterns in our bodies randomly and separately, it becomes challenging to see the overall picture. However, when we can document all our physical symptoms- both regular and occasional- on paper, we enhance our ability to self-assess. This can help us better facilitate our own healing or verbalize these patterns to other health professionals. This worksheet assists us in doing that. The top portion of the worksheet is for regularly occurring physical symptoms or ailments. The bottom portion of the worksheet contains symptoms we notice on occasion or have a history of that may need monitoring.
